The Bridge Intensive Outpatient Program is looking for a full time Peer Recovery Specialist to provide peer recovery support to program participants.Responsibilities:
  • Attend daily staff meetings and inform staff of any important participant information.
  • Implement recovery process for residents as defined PHMC’s philosophy.
  • Provide continuity of care as defined by treatment plan for each consumer.
  • Lead psycho-educational and self-help groups.
  • Engage consumer population by sharing personal story of recovery.
  • Provide outreach at treatment facilities, on the streets and consumer homes in order to ensure engagement during and prior to entering treatment.
  • Transport consumers to initial intake and for all necessary appointments.
  • Take consumers to community based self-help meetings, such as AA, NA.
  • Sit in with consumers during recovery planning meetings as his/her advocate and resource.
  • Assist with aftercare and discharge planning and maintain contact with consumers at least 2 weeks post discharge.
  • Alert clinical team to any current or potential behavioral or health related problems of participants.
  • Meet or exceed program productivity requirements.
  • Ability to work different shifts as needed.

Treatment Plan:

  • Provide input to clinical team.
Progress Report:
  • Maintain continuous record of all contacts, events, and progress toward accomplishment of goals as stated in Recovery/Treatment Plan.
  • Record individual and group progress notes in the DAP format after each therapeutic activity.
  • Complete reviews in compliance with state licensing and other program guideline
  • Maintain charts that meet all standards.
Continuing Care Plan:
  • Assist treatment team and consumers with the information of continuing care plan.
  • Other duties as assigned.
Qualification:
  • Certified Peer Specialist (CPS)
  • High School Diploma or GED
  • At least 1 year of experience working or volunteering with people with addiction and mental illness.
  • Ability to establish trusting and therapeutic relationships.
  • Commitment to recovery, choice, empowerment and the ability of people with co-occurring disorders to find meaningful lives in the community
  • Have a valid driver’s license in good standing.
  • Bilingual in Spanish Preferred
Salary:
  • Commensurate based on education and experience.

FLSA Classification: Non-Exempt

  • This position is classified as salaried non-exempt in accordance to FLSA standards.
