Job Description

The lead Public Health Nurse-Generalist (PHN-G) manages a special initiative(s) focusing on time defined two home visit model for post-partum families enrolled in local managed care organization (MCOs). This initiative concentrates on maternal/child/family well-being, social determinants of health, chronic medical and/or social conditions that predispose members for increased consumption of health and social services.  These conditions include disease management, suspected or known child neglect and abuse, Intimate Partner Violence (IPV). The PHN-G will engage member in determining present health and social status and utilize select assessments to support member/family goals, current resources and needs including influential social determinants of health, document same utilizing specific data base(s) and issuing periodic reports as required. 

There will be monthly, or as needed communication, with MCO leadership staff as well as follow-up with select MCO staff responsible for member engagement.  As a member of a multi-disciplinary team of Public Health Nurses, Managers, Resource and Data managers/staff, civil law attorneys others, the PHN-G will be responsible for ongoing communication about roles and responsibilities and client/member needs; participate in clinical conferences, group learning activities, network with community organizations, contribute to client related events/activities. The Lead PHN-G oversees other staff including one or more PHN-G and Community Health Resource Navigator(s) whose principal responsibilities are to engage with members assessed by the nurse in need of essential community and provider resources.  The Navigator can engage member via in –person home visit as well as remotely connecting by telephone or other modalities given the COVID-19 pandemic, current civic unrest and new practice protocols. The Lead PHN-G will have 20% administrative, program and staff oversight and 80% clinical practice.

Responsibilities: 

  • Provide supervision for Public Health Nurse-Generalist role and overall administrative direction for the program.
  • Manages public health nursing initiatives supporting the health and well-being of MCOs members.
  • Oversees and supports other PHN-Gs and Resource Navigators in home visitation and telehealth services related to MCOs members and/or enrolled clients in evidence-based home visitation models.
  • Provides direct clinical services as the Lead Public Health Nurse to member population using two visitation nursing model.
  • Participates in multi-disciplinary team conferences including MCO staff.
  • Implements, manages relevant data bases and submits data reports to MCO and others on regular basis.
  • Identifies relevant resources for members and maintains resource referral logs.
  • Schedules Resource Navigator follow-up with referred members (post initial nurse visit).
  • Refers to relevant organizations and services as appropriate.
  • Develops charting criteria and record maintenance regarding member status and Utilizes relevant screening tools including I-HELP tool (social determinants of health).
  • Actively contributes to events and activities relevant to MCO member.
  • Works closely with MCO senior staff to share member status, need for follow-up.
  • Contributes to overall goals and mission of home visitation models/services offered.
  • Designs innovative initiatives with others to address community/client needs post COVID-19 and current civic
    unrest related to complex interrelated challenges.
  • Refers members if eligible and interested to evidence-based home visitation models such as Philadelphia NFP, Mabel Morris Family Home Visit Program, other Philadelphia based Parents As Teachers models, Healthy Families America, Healthy Start, Healthy Homes and others.
  • Assumes other responsibilities as needed.

Qualifications:

  • Baccalaureate Degree in Nursing from accredited university or minimum of Associate Degree in Nursing and enrolled in Baccalaureate of Nursing program with target gradation date of 2021/2023. 
  • Advanced education preferred in health/social services, public health, communication or related field.
  • Current Pennsylvania nursing licensure in good standing.
  • Work clearances that include FBI, Commonwealth of Pennsylvania’s Child Clearance, Criminal Check and others required by Public Health Management Corporation.
  • If possesses automobile and uses for work related activities, has valid and current driver’s license and automobile insurance.
  • Evidence-based public health home visitation models using telehealth as needed.
  • Able to work on site or off site per assignments/commitments.
  • Flexible schedule including some evenings and weekends.
  • Sound physical condition and physically able to perform job in home and community environs.
  • Light to moderate lifting and transport of program/client materials.
  • Light to moderate exercise including walking throughout the day, stair climbing, use of transportation services.
  • Work within the County/City of Philadelphia.
  • Local to regional travel as required by program model or funders.
  • Three or more years of experience working in community/public health with urban, underserved populations highly preferred.
  • Advanced knowledge and skill base with one or more evidence-based home visitations models with 3 or more years of experience highly preferred.
  • One or more years of clinical experience in maternal-child health, early childhood, public health preferred.  New graduates and second-degree students are welcome to apply.
