The Part-time Bilingual Community Patient Navigator is responsible for conducting outreach, education, and providing navigation services to individuals enrolled in HPC's Family Planning Access to Navigation (FPAN) program. The Bilingual community patient navigator will report to the Community Engagement Manager. This position is responsible for working in collaboration with the team to implement community outreach and education and recruit participants in need of education and navigation services. The Navigator works closely with the Program Manager to ensure that project deliverables and timelines are met. Additionally, the Navigator interfaces regularly with community partners, and community residents of all ages with an emphasis on the 16-24 age group. More specifically, responsibilities include providing community education sessions on family planning; providing referrals as needed; coordinating appointments for individuals needing sexual and reproductive health services; assisting with collection of data; and attending regular meetings with program manager and team to discuss barriers/ challenges.


  • Networks and schedules community presentations for education and recruitment purposes.
  • Builds and maintains strong relationships with organizations.
  • Provides care coordination services to individuals entering the program.
  • Develops and/or implements new approaches to improve program delivery, content, and/or evaluation to better serve the targeted population.
  • Meets regularly with Program Manager to provide project updates and communicate project successes and anticipated barriers/challenges.
  • Provides updates and discusses barriers/challenges with project team members and Navigation Network members at all scheduled meetings.
  • Attends regular grantee/partnership, state-wide and national meetings, as needed.
  • Keeps program materials and supplies organized to ensure adequate inventory for program needs. Reports materials and supplies needs to Project Manager.
  • Works with Project Manager to complete all tracking reports.
  • Always maintains the highest level of confidentiality.
  • Follow all organizational, funder, and departmental guidelines.
  • Provide interpretation services during appointments, if needed.
  • Conduct appointment follow-up, if needed, including coordinating/scheduling follow-up services, and providing additional referrals for supportive services.
  • Attend all department and organizational meetings.
  • Other duties as assigned


  • Bilingual, fluent in English and either Cambodian or Korean
  • Excellent oral communication skills
  • Public speaking skills
  • Strong organizational skills, including the ability to prioritize multiple assignments
  • Availability during daytime and some evening and weekend hours
  • Willingness to travel throughout Philadelphia


  • At least two years experience working in a community navigation role, outreach, and/or as sexual reproductive health educator

Education Requirement:

  • Bachelor's degree in social work, public health, or a related field


  • Commensurate with education and experience
  • Grade 16
