The Philadelphia Hepatitis Outreach Specialist will conduct street and venue outreach throughout Philadelphia in targeted communities in partnership with other members of the HCV Street Outreach team. The focus will be on areas where there is a high level of drug trafficking and other high-risk activity to identify substance-involved women and/or men at risk for viral Hepatitis and HIV/AIDS. The main responsibility of this position will identify and address barriers to sustained HCV treatment adherence through education, screening, behavioral health service, and linkage to treatment for individuals throughout the City of Philadelphia.

Responsibilities: are to include, but are not limited to:

  • Responsible for adherence to identified Street Outreach Safety Protocols in conducting street outreach activities
  • Responsible for organizing and conducting community health fairs and facilitation of educational workshops and presentations throughout the community
  • Responsible for interviewing individuals, and documenting demographic, substance use and health information as required by program protocols
  • Responsible for to link individuals to substance treatment and primary health care
  • Responsible follow-up with participants who are screened and who are in need of linkages to primary or specialized health care or substance use treatment services
  • Responsible for making and tracking referrals for identified program participants within the PHMC Client Registry System
  • Responsible for travel throughout the city of Philadelphia
  • Responsible for meeting or exceeding specified outreach productivity targets as specified by program funders and/or PHMC program managers
  • Responsible for the documentation of all required program and participant demographic information into the PHMC Client Registry System within 72 hours outreach or contact document/record pertinent information into PHMC’s Client Registry System (CRS).

  • Strong community outreach skills
  • Ability to collaborate in a team environment
  • Strong communication skills with programs within and outside of the PHMC network
  • Experience working with the substance-involved individuals
  • Must be able to work evenings and/or weekends to support program activities

Experience:

  • Knowledge of substance use and abuse
  • Knowledge of viral Hepatitis and HIV/AIDS
  • Knowledge of public health, social service, resources and agencies in the community.
  • Knowledge of Hepatitis education, screening, and group facilitation  
  • Familiarity with project’s the target area and population

Education:

  • High School diploma or GED
  • At least two years of community outreach or locating experience
