Job Description

Position Responsibilities:
  • Work as part of the CUA Case Management Team to achieve the objectives established in each family’s Single Case Plan (SCP).
  • Provide a range of services, supports, interventions and referrals to caregivers to facilitate safe case closure for in-home cases.
  • Provide a range of services, supports, interventions and referrals to achieve reunification, adoption, or permanent legal custodianship for children and youth in out-of-home placement.
  • Provide direct assistance to CUA Case Managers to identify and pursue permanency opportunities.
  • Track and manage the movement of cases through the permanency process.
  • Support the development and progress of contingency permanency plans for each child and youth in care.
  • Assist the CUA Case Manager in preparing family members for reunification or other permanency through providing direct support, referrals, interventions and advocacy.
  • Support the CUA Case manager in preparing for court; attend court hearings as necessary in support of the permanency process.
  • Supervise visitation as necessary.
  • Participate in Family Team Conferences when necessary.
  • Document all client activity in the appropriate section of the Electronic Case Management System within four business days.
  • Abide by the Social Work Code of Ethics and maintain the confidentiality of information regarding children, youth, and families being served.
  • Participate in all other training as directed.

  • Participate in regular meetings with staff to ensure priorities are clear, coordination is good and communications are open. Develop additional methods for ongoing effective communication as necessary.
  • Cooperate and communicate as a multi-discipline staff team member through formal meetings, informal consultations and committee participation.
  • Model traits of a responsible team member, executing job responsibilities, open communication, and good follow through; support team members to do the same.
  • Demonstrate use and support of the Sanctuary Model in all aspects of employment, including interactions with children, youth, caregivers, family members, other employees, community members, and stakeholders.
  • Respond promptly to the concerns and interests of the clients, parents, guardians, funding entities and other Wordsworth staff.

Administrative activities:

  • Ensure compliance with all Wordsworth’s policies and procedures, including confidentiality and reporting suspected abuse in accordance with policies and procedures.
  • Follow all documentation requirements; ensure reports are complete, thorough, timely and follow prescribed format.
  • Prepare and deliver reports to outside agencies as appropriate; ensure reports are maintained in an accessible manner.

 Skills:

  • Knowledge and understanding of child welfare system, including PA Child Protective Services law and Adoption and Safe Families Act.
  • Knowledge and understanding of Philadelphia DHS Safety Model of Practice, including Risk and Safety Assessment and FAST/CANS assessment.
  • Knowledge and understanding of the Center for the Study of Social Policy’s Strengthening Families Framework.
  • Strong assessment and documentation skills.
  • Strong computer skills in all Microsoft office applications including data management skills.
  • Basic knowledge of safety factors related to substance abuse, sexual abuse, physical abuse, domestic violence, community violence, homelessness, poverty, etc.      
  • Strong knowledge of the communities and organizations within the CUA region.
  • Ability to effectively engage and work with diverse community groups.
  • Strong interpersonal skills.
  • Ability to clearly and effectively communicate verbally and in writing;

Physical Demands/Work Environment:

  • Social service office environment and various community environments for children, adolescents and families. Employee may be involved in de-escalating client behaviors using Wordsworth approved techniques. Potential exposure to blood borne pathogens.
  • Ability to independently utilize routine office equipment.
  • Ability to independently travel outside of agency.

Education/Credentials:

  • Must have a Bachelor’s degree in Social Work or related field including but not limited to sociology, psychology, counseling, criminal justice, education, divinity, or public health administration. Candidates with a degree in an unrelated field require a minimum of three years experience working in child, youth, and family serving systems and an approved waiver from the Department of Human Services.
  • PA Criminal Background Check, Federal Bureau of Investigation Clearance, PA Child Abuse Clearance, Wordsworth medical verification, and vehicular/traffic history (PA Driver Information or other state driver information if applicable).

Experience:

  • Two years experience working in child and family serving system preferred.

Salary:

  • Commensurate based on education and experience.
