Job Description

FLSA Classification: Non-Exempt

  • This position is classified as salaried non-exempt in accordance to FLSA standards.

AmeriCorps members will be placed at organizations in Pennsylvania to provide outreach and education around opioid use disorder to patients and their family members with the overall goal of recovery in designated counties in Pennsylvania. The main role of this position will conduct street and venue outreach, recruit, advocate, identify and address barriers to sustained adherence through education, screening, behavioral health service, and linkage to opioid treatment. The position will additionally be responsible for assisting patients in understanding and advocating for their health care needs, and navigating and accessing health and social services as necessary.

This position will be placed with the National Nurse-Led Care Consortium and work on expanding hepatitis B virus (HBV) provider and patient education, vaccine availability, and tracking screening in Public Health Management Corporation (PHMC) health centers in Philadelphia, PA.

Responsibilities:

  • Identify community linkages to support community recovery efforts
  • Provide new participants with initial outreach and orientation to the program
  • Provide outreach to participants at treatment facilities, in the community which includes recovery houses, homes and workplaces to support engagement throughout participation
  • Provide continuity of care as defined by recovery plans for assigned participants
  • Assist participants in recovery in areas such as self-esteem enhancements, conflict resolution, assertiveness and other recovery skills.
  • Alert the case management unit to any current or potential behavioral or health related problems of participants.
  • Identifies opportunities for partnership and development of relationships which could be mutually beneficial to families, the community and the agency.
  • Help patients connect with transportation resources and receive appointment reminders as necessary;
  • Assist with expansion of HBV awareness and educational materials for patients in health centers
  • Assist with educating providers on HBV vaccines, screening, linkage to care, and treatment with support of the Philadelphia Department of Public Health and Health Federation resources
  • Utilize electronic health records to track vaccine, screening, linkage, and treatment rates
  • Distribute patient surveys to evaluate current and needed cultural competency of healthcare experience at PHMC health centers
  • Comply with all regulations under HIPAA regulations
  • Maintain appropriate records of outreach activities, participants, and outcomes
  • Document all services and contacts in a timely manner in the electronic health record system and other tracking systems 

Skills/Experience:

  • Ability to communicate utilizing a recovery based problem solving approach and past experiences to support a recovery lifestyle
  • Excellent time management skills and attention to detail to attend to program target goals and to ensure timely documentation
  • At least two years of experience with vulnerable populations, including incarcerated, substance use and/or mental health disorders; knowledge of Philadelphia is a plus
  • Ability to adapt to fast paced medical environments and demonstrate flexibility in operating within different work and community settings
  • Ability to establish trusting and therapeutic relationships.
  • Commitment to recovery, choice, empowerment and the ability of people with co-occurring disorders to find meaningful lives in the community
  • Strong community outreach skills
  • Ability to collaborate in a team environment
  • Experience working with the substance-involved individuals
  • Knowledge of social services, child welfare and family systems services.
  • Familiarity with project’s the target area and population
  • Ability to work inclement weather

  • Ability to link individuals to substance treatment and primary health care
  • Ability to follow-up linked to care and provide project referrals
  • Ability to travel between surrounding Philadelphia communities

  • Culturally sensitive to the needs and diversities of multi‐cultural communities.
  • Strong interpersonal skills, respectful, and courteous nature.

Education:

  • High School Diploma or GED
  • At least two years of community outreach or locating experience
  • Some college in social work, sociology, psychology, counseling, criminal justice, education, divinity, or public health preferred
