Job Description

We are looking for compassionate and motivated nurses to work with families towards our shared goals of better pregnancy outcomes; improved child health and development; increased school readiness; and greater economic self-sufficiency for families.


  • Participates in comprehensive trainings regarding Nurse-Family Partnership (NFP) and Parent's as Teachers (PAT) program goals, theories, practice principles, rules of engagement and model elements.
  • Contributes to internal professional development seminars and weekly clinical conferences.
  • Adheres to nursing model of home visitation and community engagement.
  • Establishes and retains a caseload of pregnant women, new mothers and eligible families in accordance with NFP and PAT requirements. Visits clients in their homes on a weekly or semi-weekly basis.
  • Develops therapeutic, long-term relationships with clients utilizing concepts of reflection and motivational interviewing.
  • Supports, through one on one education, small group workshops and referral processes, the goals and achievements of women and their families.
  • Assesses and documents physical, emotional, social and environmental needs of women and their families. Records data on client needs, outcomes and milestones via computerized databases. Participates in regular review of reports and quarterly record audits for quality improvement purposes.

  • Mid-level proficiency in Microsoft Word, Excel and other relevant software programs and databases

  • Has basic knowledge of community resources and interest in learning about relevant health, education, housing, social services and early childhood programs
  • Possesses strong interpersonal skills and the desire to work in collaborative public health model
  • Multilingual applicants are especially welcomed


  • Three or more years of experience working in community/public health with urban, underserved populations in some capacity highly preferred
  • One or more years of nursing experience working in maternal-child health, early childhood, home visitation mode, and health education preferred. New graduates and second degree graduates are welcome to apply.

 Education Requirement:

  • Baccalaureate Degree in Nursing from accredited university required. Master’s degree in Nursing, Public Health, Community Systems, Maternal-Child Health or related field preferred.
