Job Description

The Forensic Intensive Recovery (FIR) Program is a prison deferral initiative that offers eligible criminal offenders substance abuse treatment in lieu of incarceration. PHMC is responsible for operating the Clinical Evaluation Unit, which evaluates eligible criminal offenders for chemical dependency; refers them to community based providers for residential, intensive outpatient and regular outpatient treatment services; and provides case management services for FIR program participants. The Office of Addiction Services (OAS) of the Philadelphia Department of Public Health funds this program. All clients must be referred through the Defender's Association of Philadelphia. The IPP is a direct seeking program, in lieu of incarceration and all referrals must come through the Defender's Association of Philadelphia.

The IPP (Intermediate Punishment and Parole) program is seeking a case manager to provide case management services to a caseload of 45 active clients with a history of chemical dependency. 

RESPONSIBILITIES
• Establish and maintain service productivity goal at minimum of designated service units per week
• Documentation of service provision within 24 hours of service completion to individuals assigned
• Participation in weekly team meetings and scheduled case conferencing with probation meeting reporting requirements
• Participation in bi-weekly supervision meeting
• Collaboration and attendance at meetings with probation/parole, treatment providers and other collaborating agencies
• Submission of documentation for reauthorization of services for individuals on caseload
• Completion of discharge summaries within timeframes specified by program
• Assist individuals to link to recovery support services including Pro-ACT recovery center, Access to Recovery, faith-based institutions, mental health services and other identified community supports 
• Weekly schedules which detail activities for each week to be completed
• Maintain client records in accordance with agency fee for service standards, managed care, and licensing requirements
• Prioritize assisting clients to enroll in treatment, physical health services, DPA, vocational services, and educational services
• Identify, coordinate and link community resources to client to promote accessibility to recovery oriented community services
• Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach
• Average of 80% work week in the field for offsite client appointments and meetings

SKILLS:
• VALID DRIVER LICENSE REQUIRED
• Excellent interpersonal skills
• Ability to meet timelines
• Excellent oral and written communication skills
• Good analytical and problem solving skills
•Bilingual fluency a plus

EXPERIENCE:
• Knowledge of criminal justice and behavioral health system
• Experience in working with databases
• Experience in direct case management services with the chemically dependent criminal justice population a plus. Experience in fee for service delivery a plus

EDUCATION REQUIREMENT:
• Minimum a Bachelor’s degree in related field and one (1) year of direct client contact and/or drug and alcohol setting, preferred
