Responsibilities:
The Outreach Specialist will focus on areas where there is a high level of drug trafficking and other high-risk activity to identify substance-involved women and/or men at risk for HIV/AIDS, viral Hepatitis and other communicable diseases. The main role of this position will conduct street and venue outreach, recruit, advocate, identify and address barriers to sustained HIV/HCV adherence through education, screening, behavioral health service, and linkage to treatment for individuals throughout the City of Philadelphia.

- Ability to collaborate in a team environment
- Strong communication skills with programs within and outside of the PHMC network
- Experience working with the substance-involved individuals
- Physical effort required associated with walking, standing, lifting and carrying light objects (25-30 lbs.)
- Amount of travel or any other special conditions or requirements:
- Local travel within Philadelphia
Experience:
- Knowledge of stages of substance involved individuals
- Knowledge of viral Hepatitis and HIV/AIDS
- Knowledge of public health, social service, resources and agencies in the community.
- Knowledge of Hepatitis education, screening
- Familiarity with project’s the target area and population
- Ability to work inclement weather
- Ability Adhere to Street Outreach Safety Protocols
- Ability to assist with community health fair
- Ability to obtain pertinent information
- Ability to link individuals to substance treatment and primary health care
- Ability to follow-up linked to care and provide project referrals
- Ability to travel between surrounding Philadelphia communities
- Ability to work evenings and/or weekends
- Ability to build and maintain effective and professional working relationships
Education Requirement:
- High School diploma or GED
- At least two years of community outreach or locating experience
