Job Description

The Connector visits the client in their home to complete an enrollment packet, a service plan and will provide ongoing case management services during all phases of the referral. The Connector will provide clients with referrals to treatment providers, work training and job placement programs, legal and community supports. The Connector reports to the Assistant Director.

Responsibilities:

  • Visit the client in their home to complete program enrollment paperwork and a biopsychosocial
  • Develop service plan with the client to identify case management needs and to address treatment, employment and disability insurance goals
  • Provide traditional case management services in client’s home, in the community and at PHMC
  • Connect the client to and interface with appropriate community resources, treatment providers, disability professionals or work training programs
  • Collaborate closely with all members of the HHA team
  • Attend all scheduled trainings and meetings
  • Complete other duties as assigned

PHMC Compliance Responsibilities:

  • Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies.
  • Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with Office of Supportive Housing, Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
  • Adhere to all PHMC Policies and Procedures
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module

Skills:

  • Bi-lingual in Spanish a plus
  • Excellent engagement skills
  • Excellent organizational skills
  • Excellent written and communication skills
  • Good computer, tablet and mobile phone skills
  • Experience with electronic medical records
  • Applicant must have a current driver’s license in good standing and reliable transportation

Experience:

  • Experience in case management or social services
  • Experience with home visits and overall comfort in all Philadelphia neighborhoods
  • Experience working with diverse populations

Education Requirement:

  • Bachelor’s degree in social work or related field

Physical Demands:

Position requires standing 1/5 or climbing stairs, walking 1/5 of the time, requires sitting 3/5 of the time, use of hands to finger, handle or feel 2/3 of the time, reach with hands and arms, under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs. up to 1/3 of the time.

Work Environment:
