Job Overview:

The Complex Care Patient Navigator will work alongside the Complex Care Nurse Manager to provide care management to patients within the PHMC Health Network who are diagnosed with or at-risk for chronic and infectious diseases. The position of Complex Care Patient Navigator will be responsible for data collection, data entry, tracking data trends, and management of data necessary for reporting to external organizations. The Patient Navigator will also be responsible for initiating targeted outreach to identified patients, engaging them in primary care and other appropriate PHMC and community services, and assisting patients to understand their healthcare needs and navigate the health care system. The Patient Navigator reports to the Complex Care Nurse Manager.

Responsibilities:

  • Work with the Complex Care Nurse Manager to identify patients who are diagnosed or at-risk for chronic, infectious diseases and engage them in treatment and other appropriate resources;
  • Maintain necessary tracking data of participants, outreach activities, and outcomes;
  • Locate and engage patients through patient outreach, coordinating outreach efforts with outreach teams across the PHMC health network, as well as resources in the community (e.g. hospitals, social services)
  • Ensure that patients obtain timely access to comprehensive and coordinated care by providing consistent communication to the Infectious Disease Nurse Manger and other members of the healthcare team;
  • Promote patients’ self-management of health conditions by engage patients in the community, modeling advocacy skills and eliminating social, cultural, linguistic, and educational barriers;
  • Document all services and contacts in a timely manner in the electronic health record system and other tracking systems; and
  • Comply with all regulations under HIPAA regulations, AACO and the PCMH Program;

Physical Demands:

  • Position requires standing/walking 1/2 of the time and sitting 1/2 of the time, use of hands 2/3 of the time, reach with hands and arms under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs. up to 1/3 of the time.

Work Environment:

  • Moderate noise (examples: business office with computers and printers, light traffic). Travel throughout the Philadelphia area, including visits to patient homes and other facilities.  Exposure to blood borne pathogens that requires use of personal protective equipment.

Skills:

  • Ability to adapt to fast paced medical environments and demonstrate flexibility in operating within different work and community settings
  • Ability to use a computer for data entry and tracking; knowledge of Microsoft Excel
  • Knowledge of some medical terminology, medical conditions (especially chronic infectious diseases) and behavioral health conditions preferred
  • Excellent time management skills and attention to detail
  • Excellent oral/written communication skills
  • Ability to represent the agency and facilitate a relationship with the community and other providers
  • Ability to maintain client confidentiality
  • Written and oral fluency in English

Experience:

  • Experience working with vulnerable populations, including incarcerated, substance use and/or mental health disorders for at least 2 years preferred; knowledge of Philadelphia is a plus
  • Experience working in a community-based setting for at least 1 to 2 years preferred
  • Familiarity working as a member of an interdisciplinary team

Education Requirement:

  • High School Diploma/GED required
  • Certification as Community Health Worker preferred

Salary:

  • Commensurate with education and experience
  • Pay Grade 14
