Job Overview:

Public Health Management Corporation (PHMC) seeks a mission-driven individual with experience working with youth to join our growing workforce development and financial wellness initiatives. The Coach will thrive in PHMC’s creative, fast-paced environment with people who are passionate about their work.

The Coach works with the Code for Success team to provide wrap-around services to youth enrolled in job training and readiness programs. This person maintains frequent contact with participants through in-person meetings, texting, phone calls, and video conferencing. This person will be responsible for intake assessments, identifying and setting goals with participants, making appropriate referrals for supportive services, and completing periodic progress assessments, including closing assessment, to document participant progress. This person will also make warm transfers for services and/or accompany participants for health and social services appointments, as appropriate.


  • Carries an ongoing caseload.
  • Assesses participant needs and strengths with input from other team members, which include PHMC and partner staff (i.e., job training and readiness staff).
  • Administers evaluation instruments, including intake and closing assessments, and maintains case notes aligning with program and evaluation requirements.
  • Develops service plans that support both program and participant goals.
  • Provides warm hand-offs and linkages for participant to services to reduce barriers to employment and job retention.
  • Meets participants in a variety of settings, including job preparation location, participant home, community site.
  • Participates in case conferences, trainings, and project team meetings.
  • Works as part of a cross-disciplinary, cross-organization project team; has the ability to lead in peer-to-peer work environment.
  • Works with the program team to assist in the delivery of other program activities.
  • Assists with program evaluation by administering evaluation tools, entering participant data.
  • Complete program reports and other written documentation as required.


  • Excellent oral and written communication skills, especially in communicating with clients.
  • Strong youth engagement and positive youth development skills.
  • Ability to handle multiple responsibilities and to work independently.
  • Familiarity with Philadelphia and/or Camden health and social service organizations.
  • Strong interpersonal skills.
  • Superior cultural competence related to race, gender, and sexual orientation.
  • Knowledge of issues related to positive youth development and trauma is required.
  • Comfortable working with youth in low-income communities.
  • Must be willing to work flexible schedule including some evening/weekend hours as requested.
  • Must successfully pass child abuse clearance and HIPAA certification.
  • Proficient with Microsoft Office, and web-based data collection applications.
  • Willingness to travel throughout Philadelphia.
  • Integrity in handling confidential and sensitive information.
  • Bi-lingual (Spanish) is a plus.


  • At least two years direct service experience working with youth, youth experiencing trauma, and/or employment readiness.
  • Intermediate in Microsoft Office suite, Windows applications, internet and email required, experience with nonprofit database software such as Apricot is helpful.
  • Experience working with LGBTQ youth is a plus.

Education Requirement:

  • Bachelor’s Degree in social work, workforce development, financial wellness, or related field.
