Job Description


  • Clinical supervision and administrative oversight of Behavioral Health Staff
  • Provide clinical supervision to all Behavior Health Consultants (BHCs) across the PHMC Health Center network, provide clinical consultation to BHCs as complicated issues arise, and be immediately available to BHCs during working hours in the event of a psychiatric emergency.
  • Provide ongoing training, documentation review, quality management (including chart audits) across the network, and performance evaluations to BHC staff .
  • Create policy and procedures related to delivery of care and care coordination as related to integration of behavioral health services
  • Serve as the PHMC representative to the Health Federation Behavioral Health Network and at the PHMC Clinical Directors Meetings; serve as a thought leader to the PHMC network staying abreast of trends, and evidence based interventions to benefit the population served
  • Interface between PHMC’s Behavioral Health Services and Specialized Health Services to support integration efforts
  • Lead and coordinate activities related to hiring and training all BHCs for the PHMC Health Center Network
  • Ensure that the BHCs are meeting the productivity targets
  • Provide oversight and direction to grant funded programs and initiatives related to behavioral health intervention
  • Serve as the interface to external agencies (DBH, TRI, TJC, HRSA) to coordinate, collaborate, and disseminate information related to behavioral health
  • Produce, review and analyze data related to grants and delivery of services

Behavioral Health Consultation in the context of primary care

  • Consultation with primary care providers to improve patient care, enhance understanding of patients, provide support for treatment planning, and assist in the implementation and monitoring of evidenced based assessments and interventions.
  • Individual, couples, family and group consultation including:
    • Functional and strength-based assessment and diagnosis;
    • Psychoeducation for patients and their support systems;
    • Medication adherence counseling and disease self management counseling;
    • Motivational Interviewing to develop behavioral strategies aimed at symptom reduction
    • Brief problem solving cognitive intervention aimed at modifying negative thinking and promoting self efficacy;
    • Self-Care Plan development and skills training to facilitate disease management, improved coping, distress tolerance, stress reduction, and relaxation; and
    • Substance use/abuse evaluation, identification of maladaptive coping strategies, and development of harm reduction strategies.
  • Coordination of Care – Provide consultation to and coordinate care of patients with health center primary care staff. Identify, refer, and advocate for patients needing specialty behavioral health service, and other services as needed.
  • Crisis Intervention -  Be immediately available to other health center staff during working hours in the event of a psychiatric emergency.  Participate as a member of the health center staff in the event of any other kind of emergency.

Population Management – Identify population health issues and work with other providers to develop comprehensive strategies to address them.

Charting – Complete all charting as needed and required by contracted funding sources: including assessments, SOAP notes, and billing.


  • Experience working in a medical setting
  • Medical literacy
  • Experience working in behavioral medicine and health psychology
  • Experience working as a member of an interdisciplinary team
  • Comfort with evidenced-based protocols
  • Must have a minimum of 1 year experience, 2 years preferred

Education Requirement:

  • Licensed Clinical Social Worker (in Pennsylvania), or
  • Licensed psychologist (in Pennsylvania)
