The Hep C, HIV & PrEP Patient Navigator will assist the Hep C and HIV care teams with program implementation. They will help patient’s secure PrEP, help with medication adherence and retention on PrEP, facilitate testing for HIV, assist as part of a team to deliver Hep C and HIV test results to patients, conduct patient follow-up, and participate in Hep C Treatment, HIV Treatment and PrEP promotional campaigns, outreach, data entry and other duties, as needed.

The Hep C, HIV & PrEP patient navigator will take the lead with those testing negative to ensure the patient is educated about safe behaviors and PrEP, receives the necessary lab work to start PrEP, and is assisted with the costs of medication so as not be a barrier to adherence and patient care.

Responsibilities:

  • Provides confidential assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services;
  • Develops PrEP/PEP resources available in the area for the clients in need of the services;
  • Develops a network with PrEP/PEP providers;
  • Provides access to prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance;
  • Provides and ensures access and active referrals to the screening of STIs;
  • Collaborates with project staff in advertising and media promotion of the Hep C and HIV testing programs;
  • Responsible for facilitating Hep C and HIV antibodies testing on-site and through outreach into the community, including other community settings where the target population socializes and/or congregate as needed;
  • Provides negative Hep C and  HIV test results to individuals who have tested for Hep C and/or HIV; (positive HIV results will be communicated by a provider)
  • Offers Partner Services to individuals who test positive for HIV;
  • Performs follow-up risk assessments with individuals who test negative for Hep C and/or HIV to reinforce the individual’s realistic perception of risk;
  • Collaborates with Pharmacist and social worker for linkage, and re-linkage to prevention services, STIs screening, and follow up;
  • Maintains communication between inter, and intra-agency services providers for successful coordination of services;
  • Maintains records of all activities;
  • Assists in documenting program activities, and reports outcomes as required;
  • Provides community outreach and education; and
  • Performs all other duties, as assigned and necessary for FHC operations.

  • Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
  • Adhere to all PHMC Policies and Procedures.

Job Requirements:

Skills:

  • Medical Terminology certification is preferred
  • Strong organizational skills
  • Proficient with Microsoft Word and Excel
  • Comfort and flexibility working in a fast-paced health environment
  • The ability to sit for the majority of the work day
  • Strong communication skills

Experience:

  • Experience as a Patient Navigator, Case Manager, Education or Human Services and great customer service experience is preferred.
  • Experience in Hep C and HIV/AIDS services is preferred.
  • Experience with the LGBTQ, minority and low-income populations.
  • Specialized training or experience in Hep C and HIV prevention interventions, PrEP/PEP, and other STIs preferable.
  • Experience working with youth and persons at high-risk for Hep C and HIV infection.
  • Experience working with substance abuse, socially, ethnically and economically diverse populations.
  • Ability to work independently as well as in a team environment. Must demonstrate sound judgement and initiative. Experience in any educator role is preferred.

Education Requirement:

  • High School Diploma or GED required.
  • Bachelor’s degree in social work, human services, pharmacy, nursing, public health or equivalent preferred.

Physical Demands:

  • Position requires sitting 2/3 of the time, use of hands over 2/3 of the time, use of talking or hearing over 2/3 of the time.

 Work Environment:

  • Moderate noise (examples: business office with computers and printers, light traffic).

Salary:

  • Grade 14

FLSA Classification: Non-Exempt

  • This position is classified as salaried non-exempt in accordance to FLSA standards.
