The registered nurse will provide primary and urgent care services to adults or families, according to certification, in community-based nurse-managed health centers to diverse and underserved populations.  Employee will report to the Clinical Director


  • Perform initial assessment evaluations for adults at health centers and outreach sites obtaining social, medical, mental health, substance abuse, and medication history
  • Collaborates, communicates and meets Medication Assisted Treatment criteria on patients as needed and required
  • Provide education and assessment to patients looking to access MAT programs
  • Provide medication management using specified guidelines at health centers and outreach sites
  • Evaluate and treat patients in accordance with clinical best practices for urgent care and primary care setting.
  • Provide ongoing management, education, and support of patients in all phases of treatment including; nursing visits scheduled and random, routine labs and as needed, medication teaching, monitoring, pill counts, observed dosing, and medication refills.
  • Exhibit non-judgmental, empathetic and supportive approach when communicating with patients/families and staff
  • Incorporated evidence-based practices as appropriate for the individual client
  • Work within all state and federal laws and regulations and the agreement with the collaborative physician
  • Administer medications, provide vaccinations, and render treatment to patients
  • Manage acute and chronic health problems using evidence-based protocols.
  • Provide health screenings, immunizations and other disease prevention programs.
  • Notify immediate supervisor of significant problems.
  • Provide age-appropriate assessment and interventions in order to meet the needs of patients and families served.
  • Provides patient/education, health counseling, and health advocacy services to patients and significant others.Be willing to work evenings and weekends, as deemed necessary.
  • Maintain positive relationships with funding sources and community organizations.
  • Complete appropriate documentation with record keeping
  • Consistently use clear, concise, and effective written and oral communication; and comply with departmental policies when sharing/documenting relevant patient care data
  • Perform other related tasks and duties, as directed and assigned.
  • Act in the capacity of a mandated reporter to both the State of Pennsylvania and the Joint Commission.
  • Meets Credentialing Requirements as stipulated by Medicare, Medicaid, and private insurers


  • Ability to engage resistant clients
  • Extensive knowledge of current health care needs of diverse populations in an urban environment.
  • Can multi-task between answering calls from patients and assisting providers
  • Must be friendly, responsible, and professional
  • Must have the ability to effectively work with company employees, outside contacts, and a diverse client population.
  • Knowledge of Electronic Medical Records preferred, but not required.
  • Ability to collect adequate client data, conduct a physical exam, interpret physical findings and manage or refer patients as conditions or protocols dictate
  • Excellent verbal communication and writing skills
  • Ability to prioritize and organize work day to meet changing needs of program and clients
  • Ability to work within a team framework and respond positively to team members concerns
  • Ability to track work to completion and compile reports
  • Thorough knowledge of primary care and urgent care best practices
  • Bi-lingual preferred


  • Experience in primary and/or care, community health preferred. Certified as a Registered Nurse in the State of Pennsylvania. Three (3) years of nursing experience. Medically Assisted Treatment (MAT) preferred.


  • Education: Nursing degree from an accredited program required
  • Licenses: Valid Pennsylvania RN


  • Commensurate with education and experience
