The Program Coordinator is a full-time position with the Training & Capacity Building (TCAP) department and reports to the Program Manager. The TCAP department serves as a community-clinical integration hub for delivering, scaling and sustaining evidence-based chronic disease self-management and prevention programs for at risk populations through its local, regional, statewide and national multi-sector partnerships. Currently these programs target diabetes, pre-diabetes, chronic disease, chronic pain and arthritis self-management among high risk disparate populations, such as African American and Latino/Hispanic populations and those living with disabilities and behavioral health conditions. Ultimately, TCAP’s work aims to simplify and increase access to evidence-based self-management programs.

The Program Coordinator is responsible for planning, implementing, and performing a wide range of activities and tasks to achieve the goals of an assigned portfolio of services and programs based upon grants and contracts of the department. This requires the coordinator to develop and maintain a thorough knowledge of the evidence-based programs within TCAP’s portfolio, as well as a comprehensive understanding of the partner network and target audiences within the assigned region(s), in order to expertly recruit and engage them in TCAP’s services and programming. Required is an ability to adapt oral, written, and nonverbal communication in a clear, professional and inclusive manner to a variety of audiences, including but not limited to, diverse community members;  staff and stakeholders of community based organizations; health care and health plan executives and network providers; clinical and care management teams; health educators; and state and federal government agencies.

As with all positions, the Program Coordinator role is contingent upon funding.

Responsibilities:

  • Oversee daily operations, establish priorities, and coordinate the activities of assigned project
  • Plan, develop, implement, coordinate and track promotion and outreach to the community and partners.
  • Develop and maintain knowledge on evidence-based programs, as assigned, under TCAP’s portfolio with respect to program delivery structure, requirements, participant eligibility, and partners delivering such programs in the community and statewide.
  • Depending upon funding and Coordinator expertise and training, conduct a limited number of EBPs with target audiences either in-person or virtually/remotely.
  • Develop and implement work plans, as assigned by Program Manager, to meet deadlines of contractual agreement requirements of funded projects.
  • Establish and maintain relationships with internal and external partners and stakeholders to serve as a primary point of contact for evidence-based program providers.
  • Deliver technical assistance to community-based and clinical partner organizations.
  • Develop and disseminate promotional materials and tools increase awareness and utilization of partners to either deliver or refer to evidence-based self-management and prevention programs.
  • Identify target audiences and conduct outreach to recruit and enroll participants into EBPs.
  • Routinely identify resources, webinars, and new literature to assist the TCAP team in staying current with chronic disease, diabetes-related news, and circulate the relevant resources on evidence-based programs.
  • Coordinate and facilitate meetings.
  • Compile and analyze data from a variety of activities and data sources for planning and reporting.
  • Complete monthly, quarterly, semi-annual and annual reports according to grant or contract requirements.
  • As assigned, supervise interns, volunteers, part-time casual staff, and/or AmeriCorps VISTA.
  • As assigned, develop and complete submissions for national, statewide and local public health conferences to highlight HPC’s work (poster or oral presentations).
  • Assist in the preparation of grant requests and funding continuation.
  • Learn and utilize HPC’s server for storing and retrieving files.
  • Obtain HIPAA training within 3 months of employment.
  • Other duties as assigned.

Skills:

  • Possess strong oral and written communication skills, including public speaking and presentations.
  • Must be detail oriented with scheduling, communication, document formatting, proof-reading for grammatical and typographical errors.
  • Strong interpersonal communication with ability to tailor communication to diverse audiences.
  • Demonstrated ability to work well independently and within a team setting with minimal supervision.
  • Be able to establish and manage priorities, and efficiently meet personal, team, and partner deadlines.
  • Must be proficient in Microsoft Office, including: Outlook, Word, Excel, PowerPoint, Publisher and have an ability/desire to learn new software, as needed or assigned.
  • Have or develop proficiency in social media platforms such as Facebook and Twitter to increase HPC’s visibility.
  • Strong problem-solving abilities.

Qualities:

  • Self-motivated, hard-working, and dependable, and thrives in a fast-paced environment
  • Must have strong ability to multi-task, juggle priorities and meet competing deadlines
  • Must have ability to work outside “normal business hours”; limited evenings and weekends may be required.
  • Must be willing to travel up to 10% of the time outside of the local area, which may require overnight trips

Experience:

  • Two years of experience in public health or related program delivery and/or coordination among multiple stakeholders.
  • Experience working with diverse populations, as well as low-income individuals.
  • Experience working with grant funded programs, particularly focused on chronic disease management and prevention.

Education Requirement:

  • Bachelor’s degree or equivalent work experience in public health, health policy, management, health education, communication, or management or a related field of study; Master’s Degree is a plus, but not necessary.

Salary:

  • Commensurate with education and experience
  • Grade 18
