PHMC’s Forensic Services Department is looking for a full time Peer Recovery Specialist to provide peer recovery support to Intermediate Punishment Program case management unit within Forensic Services. This Peer specialist will work with the IPP Case management supervisor to provide services to participants in the IPP program. The IPP Peer Recovery Specialists will assist the participant and their assigned IPP case manager with their recovery goals.
Responsibilities (Included but are not limited to):
Provide regular face to face meetings and phone contacts with participants to identify strengths and challenges to support recovery
Identify community linkages to support their recovery efforts which includes job readiness, educational aspirations, housing and family support
Provide additional support to participants who find treatment compliance challenging.
Provide new participants with initial outreach and orientation to the program
Provide outreach to participants at treatment facilities, in the community which includes recovery houses, homes and workplaces to support engagement throughout participation in Forensic Service Program.
Attend case management team meetings and compliance trainings as scheduled
Engage participants by sharing personal story of recovery.
May attend case management plan meetings with participant and assigned case manager.
Provide continuity of care as defined by recovery plans for assigned participants.
Assist participants in recovery in areas such as self-esteem enhancements, conflict resolution, assertiveness and other recovery skills.
Meet participants at community based appointments such as self-help meetings ( AA, NA), physician appointments, vocational services, etc...
Alert the case management unit to any current or potential behavioral or health related problems of participants.
Provide comprehensive wellness support to participants which includes routine primary care and mental health services (as needed)
Document participant service provision into participant registry system as outlined by PHMC meeting deadlines for compliance.
