The Data Entry Specialist will work as part of the Opioid Use Disorder (OUD) Medical Home, also referred to as Center of Excellence (COE). This COE has been created by the Commonwealth of Pennsylvania to help address systematic factors that contribute to problematic use of opioids in the community. Duties will include working with a care management team to track a variety of information and state-specified measures, including patient referrals and ongoing engagement with both the CBCM team and substance use treatment providers. The Data Entry Specialist will work with the CBCM team to effectively monitor and report patient contact with the COE program components on a monthly basis. The Data Entry Specialist will be responsible for the maintenance of the COE reporting spreadsheet as well as assisting with maintenance of other tracking tools and documents, as needed. The Data Entry Specialist reports directly to the COE supervisor.


  • Maintain the Department of Human Services-provided Quality Measures tracking spreadsheet for the Center of Excellence program that records care management contact and engagement, and outcomes for each individual receiving Center Of Excellence services and supports.
  • Track and report quality outcome indicators as defined by the Department of Human Services and best practice guidelines at an individual and aggregate level.
  • Submit an encrypted, password-protected electronic care management and quality tracking report to the Department of Human Services.
  • Assist in the monitoring of and data-extraction from COE intake, engagement, and encounter documentation, as well as electronic health record (EHR) documentation to compile reporting measures.
  • Provide trainings related to quality metrics, as needed to Center of Excellence staff.
  • Facilitate the dissemination of the COE Patient Satisfaction survey measuring the quality of life and movement towards recovery.
  • Additional duties and tasks related to report and documentation maintenance, as needed.


  • Ability to communicate effectively and to work productively as part of an interdisciplinary team with health center staff, clients, and outside organizations in a positive, pleasant, professional and productive manner.
  • Ability to learn quickly and to develop and master new skills.
  • Ability to navigate different documentation systems, including electronic health record systems, and interpret information into defined report measures.
  • Proficiency in using a personal computer, including Microsoft Office Suite, Outlook, and the Internet; ability and comfort to work with electronic health record data systems.


  • Previous data entry and health record system navigation preferred.


  • Bachelor’s Degree required or equivalent/related work experience

  • Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards
  • Adhere to all PHMC Policies and Procedures
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module
