The  CUA  Aftercare  Worker  provides  follow  up support  and services  to families who  have  achieved  safe case  closure  after  receiving  formal  in-home  child  welfare or  child protection services.


  • Work with families whose children and youth have been reunified and then achieved safe case closure 
  • Participate in the development of the Aftercare Plan; take primary responsibility for carrying out the services and supports identified in the plan
  • Work with children, youth, and families who have achieved other permanency having particular knowledge of and proficiency around available SWAN
  • Provide a wide range of supports, services and referrals to help ensure that families maintain safety, stability and well-being for their children 
  • Utilize the Center for the Study of Social Policy's Strengthening Families framework in the course of implementing the Aftercare
  • Develop effective partnerships with a wide range of community providers and stakeholders, including behavioral health, educational, recreational and medical providers, to help family members maintain permanency and achieve the goals of the Aftercare
  • Meet with families at times and locations that are convenient to them and conducive to achieving the goals of the Aftercare.
  • Participate in Family Team Conferences, meetings and all other conferences when necessary particularly if the purpose of the conference is determining safe case.
  • Document on forms prescribed by the CUA all activities performed within six business days.
  • Work cooperatively and collaboratively with other members of the CUA Case Management Team and DHS and Juvenile Probation staff as indicated. Communicate regularly and respond  promptly to messages.                    •
  • Actively participate in all required training and complete a minimum of 40 hours annually 
  • Perform other duties as assigned by CUA Director or designee Document any activity they have perform
  • Perform the duties and responsibilities of the Positive Parenting Program (Triple P) with fidelity.
  • Keep their certification up to date  with the requirements of Triple P International
  • Maintain a caseload of families within CUA 10 to execute the Positive Parenting Program
  • Meet monthly with peers and practitioners to discuss Triple P

  • Participate in regular meetings with staff to ensure priorities are  clear,  coordination  is  good and communications are open. Develop additional methods for ongoing effective communication as
  • Cooperate and communicate as a multi-discipline staff team member through formal meetings, informal consultations and committee

  • Strong assessment and documentation
  • Strong computer skills in all Microsoft office applications including data management
  • Basic knowledge of safety factors related to substance abuse, sexual abuse, physical abuse, domestic violence, community violence, homelessness, poverty,
  • Strong knowledge of the communities and organizations within the CUA
  • Ability to effectively engage and work with diverse community groups.

  • Two years’ experience working in child and family serving system


  • Must have a Bachelor's degree in Social Work or related field including but not limited to sociology, psychology, counseling, criminal justice, education, divinity, or public health Candidates with a degree in an unrelated field require a minimum of three years’ experience working in child, youth, and family serving systems and an approved waiver from the Department of Human Services.
  • PA Criminal Background Check, Federal Bureau of Investigation Clearance, PA Child Abuse Clearance, medical verification, and vehicular/traffic history (PA Driver Information or other state driver information if applicable).


  • Commensurate with education and experience
  • Grade: 16 

FLSA Classification: Non-Exempt
