Provision of nursing care services and care management to women with children (birth-12 years of age) participants at Interim House West.

These responsibilities will include an array of assignments under the direct supervision of the Program Director.

Specific:

  • Provide professional nursing services including care case management, health education, counseling, supportive and restorative care to participants and their children. Coordinate participant’s medical and psychological needs with the medical and therapeutic staff from assessment to diagnosis and treatment.
  • Conduct within the first day of admission the comprehensive nursing assessment and schedule the participant for a complete physical examination. The medical appointment may be scheduled with the participants Primary Care Physician (PCP), or if the participant agrees, PHMC Care Clinic will provide ongoing medical services.
  • Coordinate /schedule the physical examination within 7 days of admission. This requirement is based on the Department of Drug and Alcohol Programs (DDAP) standards.
  • Coordinate children services based on their health status and emergent needs during their stay at IHW. A brief assessment should be completed on all new pediatric admissions, or request a recent physical examination results and immunization history status from their PCP.
  • Coordinate external and internal heath needs of all participants. Internal health needs provided include the Care Clinic providing a Nurse Practitioner to IHW once a week to offer onsite PE’S and Gyn examination services. Most follow/up services are provided at the Care Clinic. The Care clinic will provide health care to the entire family.
  • Participants may decide to continue their health care needs with the existing PCP. Therefore, the nurse will follow established protocols for external referrals (initiate the offsite consultation visit form; notify the assigned therapist when their participants are offsite receiving medical services.
  • Organize and maintain onsite Psychiatric services; this includes notifying the Psychiatrist of participants to be schedule during his next session at IHW. Once a Consultation Request is received from a therapist the participant is added to the doctor’s schedule. The nurse is responsible to follow through on the psychiatrics orders and notify staff of the visit results until the Initial psychiatric Assessment or Evaluation is available for staff review.
  • Coordinate health care needs of participants with their therapist and treatment team.
  • Coordinate services for pregnant women this includes prenatal, delivery and post-partum care while in treatment. Protocols are in place to assure each participant’s has a safe and health delivery.
  • Medication management is an integral aspect of the participant’s treatment/recovery and care while at IHW. Therefore, the nurse is responsible for observing the participants self-administration of medications. IHW provides Medication Assisted Treatment (MAT).The nurse will adhere to the established protocols for this specific medication service.  
  • Coordinate all participants’ prescriptions services with Tepper Pharmacy. Complete the Medication Profile form for all new participants at admission. Assure that all medical, demographic, and insurance information is accurate and submitted prior to sending new medication prescriptions to Tepper. Coordinate with case management staff to access client’s benefit information.
  • Coordinate medication services with Tepper Pharmacy and the assigned Pharmacist. The nurse will acquaint her/his self with the medication policies and procedures. The nurse will manage the receipt and utilization of the monthly Medication Administration Report (MAR), for all participants prescribed routine and MAT medications. Adhere to all medical/ medication documentation requirements.
  • Coordinate patient education sessions weekly, Prepare and update health education curriculums as needed. Coordinate any outside resources providing health education to participants’ individually/ group settings. Provide in-service education to staff when necessary regarding specific health issues /topics.
  • Conduct Urine Drug Screens randomly and after specific client activity (e.g., returning from offsite activity, changes in behavior or affect, etc.).
  • Coordinate medical on-call responsibility 24/7 with LPN, conduct monthly chart reviews for medical section of the participant’s clinical chart. Complete PIE Nursing Notes as necessary.
  • Obtain CPR and First Aid Train the Trainer certification, and organize staff CPR and First Aid training to maintain IHW in compliance with DDAP, and CBH continuing education standards. Offer infant CPR training to new mothers.
  • Establish a good working relationship with community health and service providers as well as local health departments.
  • Supervise medical support staff MA or LPN, ensure RA‘s are knowledgeable and follow observation of participants self-administration of medication and documentation of medication doses. Maintain Standard Precaution protocols among staff and participants.
  • Coordinate all infection control activates and report findings monthly to TQM.
  • Ensure that medical suite/ department is in compliance with stakeholder’s regulatory standards and guidelines. (e.g., Medication Cart audits, etc.), specifically, DDAP, CBH.
  • Liaison to contract pharmacy, laboratory and supply vendors. Receive, review and forward appropriate invoices to the appropriate PHMC department for reimbursement.
  • Coordinate with Facility manager to ensure Reportable Incidents are properly reported recorded, and all appropriate agencies and follow-up investigations are completed accurately for medical issues.

Qualifications: Minimum:

  • RN, (BSN preferred) Graduate from a certified school of nursing, and licensed in Pennsylvania. Experience in public health or community health nursing.  
  • A minimum of 2-3 years, work experience in any of the following areas e.g., Mental Health services, residential treatment programs, knowledge of reproductive and women’s health issues. Pediatric nursing experience a plus.
  • Experience, knowledge of working with culturally diverse populations.
  • Basic understanding of medical and psychosocial causes relevant to chemical dependent individuals.
  • Ability to work with and educate non-medical staff and participants.

Requirements:

  • RN licensed in Pennsylvania
  • A valid Pennsylvania Driver’s License

Salary:

  • Commensurate based on education and experience
