Job Description

The Population Health Outreach Specialist will work as an integral part of an interdisciplinary team, using Managed Care Organization reports and practice registries to proactively identify patients due or over-due for specific health services, contacts the patients, motivates the patient to complete the recommendation, and assists in scheduling appointments. The Outreach Specialist is a valuable resource for analyzing provider panels, care gap reports, and providing outreach to patients overdue for services. It’s essential that the Outreach Specialist maintain a patient focus, shares a passion for patient-centered care, and assist in meeting organizational and efficiency goals. The candidate must possess the ability to work independently with strong organizational, communication and interpersonal skills, with significant attention to detail for completion of both verbal and written external communications.


  • Responsible for contacting new and established members identified on insurance lists and scheduling appointments in appropriate time frames. Also work with insurance companies to remove members who have a PCP outside of PHMC.
  • Work directly with the Revenue Cycle Manager regarding insurance quality programs and care gaps.
  • Contact all patients who are indicated on the Care Gap Report for managing various Managed Care Organization and internal patient reports and data, including but not limited to— care gap reports, new member reports, and lost-to-care reports
  • Contact all patients who are indicated on the Care Gap Report as “Due or Over Due” for a specific condition.
  • Complete Intensive Case Management Worksheets for chronic and or complex care patients
  • Obtains all necessary appointment information and assures patients are scheduled appropriately for evaluation, checking with clinical staff when necessary for guidance.
  • Maintains and updates patient demographic and insurance information as appointments are scheduled
  • Answers patient’s questions regarding appointments or testing, addressing potential barriers to care
  • Help patients connect with transportation resources and receive appointment reminders as necessary;
  • Perform other duties as necessary.

  • Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
  • Adhere to all PHMC Policies and Procedures.

  • Medical Terminology certification is preferred
  • Strong organizational skills
  • Proficient with Microsoft word and excel
  • Comfort and flexibility working in a fast-paced health environment
  • The ability to sit for the majority of the work day
  • Strong communication skills


  • 1 year health care experience required
  • Minimum 1 year customer service experience preferred

Education Requirement:

  • High school diploma or GED 

Physical Demands:

  • Position requires sitting 2/3 of the time, use of hands over 2/3 of the time, use of talking or hearing over 2/3 of the time.

Work Environment:

  • Moderate noise (examples: business office with computers and printers, light traffic).
