Job Description

The Infectious Disease Nurse Manager oversees programs intended to treat patients with higher acuity primary care needs across the PHMC Health Network. These programs include but are not limited to the treatment of HIV, Hepatitis C, and HIV Pre-Exposure Prophylaxis. The Nurse Manager oversees a panel of patients, providing medication management and care coordination efforts in collaboration with treatment providers. The Nurse Manager seeks to improve health outcomes and increase treatment adherence for patients diagnosed with or at risk for chronic, complex diseases.

Additionally, the Nurse Manager is responsible for supervising the Hep C, HIV and Prep Patient Navigator, as well as managing the data collection relevant for reporting to external organizations and funders. The Nurse Manager reports to the Quality Manager.


  • Identifies patients who are diagnosed or at-risk for chronic or infectious diseases and engage them in treatment and other appropriate resources;
  • Oversees patient care activities, care coordination, and patient engagement in treatment
  • Performs patient assessments and screenings, provide health education, and ensure continuity of care;
  • Monitors and helps to improve quality metrics related to ACCO.
  • Supports building HIV care across all of the health centers;
  • Provides oversight for the Hep C and HIV Ending the Epidemic Grant.
  • Collaborate with providers regarding patient care and progress;
  • Provide oversight for infection control practices across the health centers;
  • Works with treatment providers and pharmacies to manages the ordering, storage and administration of medication
  • Utilizes clinical knowledge to apply evidence-based practice guidelines and criteria;
  • Document all services and contacts in a timely manner in the electronic health record system and other tracking systems
  • Maintain necessary tracking data of participants, outreach activities, and outcomes; and
  • Comply with all regulations under HIPAA regulations, Joint Commission, HRSA and AACO;
  • Maintains positive relationships with funding sources and community organizations
  • Participates in the HIV CQI meeting held monthly
  • Performs other tasks as assigned

PHMC Compliance Responsibilities:

  • Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies.
  • Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
  • Adhere to all PHMC Policies and Procedures.
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module.

  • Ability to adapt to fast paced medical environments and demonstrate flexibility in operating within different work and community settings
  • Ability to use a computer for data entry and tracking; knowledge of Microsoft Excel is a plus

  • Current CPR certification


  • One-year experience as a nurse in primary care, community health and/or behavioral health
  • two years or more experience working with HIV required
  • experience working with Hep C patients preferred
  • Case management experience preferred

Education Requirement:

  • BSN Required.
  • Pennsylvania RN License Required

Physical Demands:

  • Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting under 1/3 of the time, use of hands to finger, handle or feel 2/3 of the time, reach with hands and arms, under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs. up to 1/3 of the time.

Work Environment:

  • Moderate noise (examples: business office with computers and printers, light traffic). Travel throughout the Philadelphia area, including visits to patient homes and other facilities. Exposure to blood borne pathogens that requires use of personal protective equipment.
