A key strategy of the federal Ending the HIV Epidemic initiative is addressing persistent disparities across local systems intended to identify and prevent HIV. Rates of new HIV diagnoses in Philadelphia remain disproportionately high among Black and Hispanic/Latino gay, bisexual and other men who have sex with men, youth ages 18 to 24, transgender persons and persons who inject drugs. To minimize gaps across networks of care, PDPH designed DExIS (Demonstrating Expanded Interventional Surveillance: Towards Ending the HIV Epidemic in Philadelphia). The program builds upon PDPH’s existing surveillance and disease investigation capabilities to identify and address missed opportunities for HIV prevention.
DExIS involves several components, including: 1) utilizing public health data to identify sentinel cases of individuals with evidence of recent HIV acquisition; 2) conducting standardized, confidential interviews and medical chart abstractions of sentinel cases to determine missed opportunities for identifying and preventing HIV; 3) determining patterns of missed opportunities through a monthly inter-disciplinary Case Review Team; 4) devising actionable recommendations based on identified themes and aggregate data by a quarterly Community Action Team; and 5) mobilizing executive leadership across Philadelphia’s institutions to implement recommendations through a quarterly Policy Implementation Team. The program aims to engage individuals with lived experience and healthcare providers equally at the decision-making table.
The DExIS Chart Abstractor/Interviewer is responsible for data collection, entry and presentation, as well as identifying individual client needs and facilitating linkage to PDPH’s Client Services Unit as needed. The position will report to the DExIS Project Coordinator.
