Job Description

The Data Entry Clerk will work with EFA, Food Voucher & Transportation coordinators to enter and reconcile client level data in the Careware database. The duties require access to confidential information held by PHMC. The Clerk reports to the Transportation Coordinator.


  • Responsible to maintain the confidentiality of client level information at all times both at work and off-duty.
  • Perform data entry tasks in Careware, keying client level data in accordance with prescribed protocols and guidelines for data quality and integrity. The data includes but is not limited to client applications, registration forms and transportation invoices.
  • Responsible for reconciling the transportation activity data entered in Careware from data reports and invoices received from medical case management and transportation providers.
  • Responsible for identifying problems on verifying the eligibility of clients, and recording the problems on appropriate spreadsheet forms.
  • Responsible for preparing the Careware transportation data reports for purchase requisition; cover client names with Post-It Cover Tape to safeguard confidentiality.

  • Able to perform routine and repetitive keystrokes correctly 80%-90% of the time.

  • Able to organize and meet deadlines.


  • 1 year of data entry

Education Requirement:

  • High School diploma or GED required


  • Commensurate with education and experience
  • Grade 11

FLSA Classification: Non-Exempt

  • This position is classified as salaried non-exempt in accordance to FLSA standards.
