Job Description

The Case Management Supervisor/ Outreach Liaison is responsible for providing day to day supervision and monitoring the activities of the New Pathways Program-based Case Manager and Outreach Case Manager.

Responsibilities:

  • Responsible for the supervision of New Pathway Project Case Manager, including documented regular supervision meetings
  • Responsible for monitoring the Outreach team, including weekly field visits
  • Providing outreach support, wellness checks and education to build, connect and sustain a recovery community of participants.
  • Develop recovery-related content for groups in partnership with project staff and program participants
  • Conduct individual education and recovery support sessions to partner with program participants to assist in strengthening their self-determination and personal paths to recovery
  • Work collaboratively with program staff to facilitate participant enrollment into substance use treatment as they request.
  • Maintain careful documentation of all service activities, including use of the integrated health platform or other identified program databases.
  • Navigate various support systems including housing, substance abuse treatment services, child welfare and other systems and serve as an advocate with and for the participants to address both their short and long term recovery needs
  • Provide administrative support as needed to case management staff
  • Conduct performance appraisals and implement staff development activities
  • Provide direction, feedback and coaching to case management staff
  • Ensure service provision is implemented to program participants in accordance to agency/funder requirements
  • Ensure timely documentation of all case related activities including case management services plans and DAP notes
  • Reward and recognize outstanding employee performance on a regular basis
  • Provide data on participant services as needed by NPP Program Director
  • Oversee the implementation of pre-treatment intervention including individual case management sessions, psycho-educational and peer support group activities
  • Partner with project staff and participate in new program development activities to support and expand community-based services
  • Partner with other PHMC programs, including those within the Community Engagement Division and PHMCs Behavioral Health Network, to align and coordinate, recovery support and facilitate integration of services across departments
  • Work with other program staff to identify and participate in program-related activities including health fairs and other community events

Skills:

  • Ability to provide educational activities on the individual and group level related to substance use, mental health, health care and other recovery support related activities
  • Knowledge of substance use and other behavioral health issues including treatment modalities and related recovery support services
  • Knowledge of community-based resources and program to link at-risk individuals in need of health, behavioral health and social services

Experience:

  • Minimum five years of post-BA degree experience in community-based public health or social service programs serving low income or at-risk populations
  • Experience working with substance use populations, especially minorities and high-risk populations
  • Knowledge of trauma and its impacts on the individual and the community
  • Ability to handle multiple responsibilities and to work independently
  • Ability to work with program databases to document services as well as other program related activities
  • Provide program service and outcome data to Program Director and BHS Senior Leadership as requested
  • Complete presentations at meetings, treatment providers and other referral and collaborating partners in conjunction with the NPP Program Director
  • Excellent oral and written communication skills
  • Strong program planning and implementation skills
  • Strong analytical and organizational skills    

Education:

  • Bachelor’s degree in social work, public health, or related field required. Master’s degree preferred.
  • Certification in addictions counseling a plus
